Who approve or evaluate the Business case and feasibility study
Business Case is approved by the sponsor who is expected to spend money for the initiative. A project is initiated means its business case is approved and agreed to be funded.

Which output is deliverable and which one is work product varies project to project. Deliverables are promised to deliver to customer and Business Analysis can help to understand different output categories.

Answer is correct - At the end of each iteration

We planned budget for a release and monitor frequently - how are we progressing (at the end of each iterations)?
From couple of iterations, we start getting trend how we are progressing iteration by iteration for a release.

There is no need of lag in current sprint execution. Team and PO sees how much time is needed to refine the backlog and estimation. The team reserves time for it within Sprint.

Scrum guide say on an average 10% of Sprint time goes in such activities.
