Saket's reply: there is nothing about collecting requirements stated in the question. and normally we use prompt lists or lessons learned to generate risks for a current project from previous projects.

Saket's reply to first question: Normally you would check to see if its a common practice in a country and then you can check the resumes. Some countries can still be un-evolved and hire family members even if they don't have merit.

Saket's reply to second question: pmbok normally tries to encourage planning and not doing work until planning has been done. the pm can halt the project for an hour to create the wbs (not necessarily stop the project completely).
