Hi
If stakeholder is not happy with the project outcome , it is recommended to to log it as issue.
Than what is change request and what is issue log ? IS these two documents connected or same document with different names?

Issue Log is to record issue and Change Request is to initiate Change Management Process. Not all issues will result in initiation of Change Management Process.
Examples of Issues are:
1. Person took unscheduled leave
2. We have high number of defects that expected
3. Client has not approved our deliverable for more that 10 days.
Change Request is initiating the process of replanning when the things has high impact and as per change management plan.
Examples of Issues are:
