Clarification for Issue Log

When we need to log an issue in the issue log?
What are all the criteria?
Can we log internal team issues/ conflicts in the issue log?
Do we need to log personal issues of a team member which can affect the performance of project.

We need to register all issues in issue log as long as it not sensitive and should be restricted to select few stakeholder

There are no specific criteria, it depends on how important is the issue and its potential impact on the project

Can we log internal team issues/ conflicts in the issue log?
Usually no if it has to mention names , PM has to be pragmatic in this regards

Do we need to log personal issues of a team member which can affect the performance of project.

Thank you so much Mr. Manish for your valuable reply. I had encountered through a question asking...

If a key team member planned for a leave and he noticed in project schedule that during his absence (planned leave) some critical activities are planned which is assigned for him. Since it is a specialist job, there is no other to replace him. He cannot postpone the leave. He report this to PM. As a PM what should he do? Options are:

A. Inform Management
B. Log the issue in issue log
C. Inform Sponsor

I dont remember the last option. I think it was irrelevant.

Opion B "Log the issue in issue log" will be the correct answer?
