ISSUE LOG VS LESSONS LEARNED

Saket pls clarify, Question No. 44, Mock test 1.

A previously identified technical issue has been resolved and the project is moving forward. The project manager has been warned that the same issue will probably arise on similar projects in the future. What should be done first when responding to this warning?
A• Create a risk report and keep the report current.
B• Notify the project sponsor about the warning.
C• Ensure that the issue log is updated.
D• Ensure that the lessons learned register is updated.

My answer is D, where as correct answer is C
The technical issue has been resolved, so lessons learned register to be updated to be used as repository for future use. Issue is registered in issue log for better tracking and taking to logical end, updating it will not help other similar projects

Does Issue log also becomes part of OPA

Here is the trick
What should be done first when responding to this warning?
So anything goes wrong, you raise an issue and when it gets resolved you mark the issue resolved, once done, you update the Lesson Learned.

So it is just about the sequence of activities.
